What to Bring to Your Appointment:

✔ Any recent X-rays from a previous dentist (if available)
✔ A list of any medications you are currently taking
✔ Dental insurance information, including any required authorization forms, to help us process your claims efficiently

Important Notes:

• Patients under 18 must be accompanied by a parent or guardian.
• If you have a medical condition such as diabetes, high blood pressure, artificial heart valves/joints, or a history of rheumatic fever, please inform us before your appointment.
• Let us know if you are taking medications like blood thinners, aspirin, or heart-related prescriptions.

X-Rays

If you already have recent X-rays, you can request your dentist or physician to send them to our office. If additional X-rays are needed, we can take them during your visit for a complete evaluation.
